**Role Summary**
----------------
The Remote Supervising Provider plays a crucial role in ensuring the success of PocketRN’s dementia care (GUIDE) program. Working closely with the Chief Medical Officer, provider liaisons, clinical teams, care navigators, and patients, the Provider will review clinical documentation, complete CMS\-required attestations, and support timely, high\-quality dementia care. This role helps ensure that patients and caregivers have access to the services and support they qualify for.

**Key Responsibilities**

### **Clinical Responsibilities**
* Act as a supervising/collaborating physician for PocketRN Nurse Practitioners and, perform quarterly chart reviews, hold virtual meetings with assigned NPs as needed, etc.
* Review and sign off on documentation from care navigators and nurses in a timely manner.
* Enter relevant charges into the electronic medical record (Athena) for services provided.
* Complete provider attestations as required for the dementia care program(s).
* Provide oversight to care navigators, nurses, and APRNs supporting program beneficiaries.
* Respond to escalations from clinical teams and care navigators as needed.
* When appropriate, evaluate and diagnose patients with dementia when extenuating circumstances prevent assessment through their existing care team.
* Communicate clearly and frequently with patients, caregivers, PocketRN staff, and partners.
* Follow escalation protocols for urgent scenarios and ensure timely communication with relevant parties.
* Maintain accurate, timely, and thorough documentation of patient data and clinical decisions.
* Maintain strict adherence to HIPAA and privacy standards in a virtual care environment.
### **Telehealth \& Administrative Responsibilities**
* Maintain an active, unencumbered provider license in each state where patient care is delivered.
* Review and sign off on completed comprehensive assessments, medication reconciliations, and required attestations within 7 days of the patient visit as the billing provider. This applies to p
