Join a team of seasoned clinicians and experienced professionals who deeply care about making life\-long connections with patients and redefining episodic disease care to true health care Job Summary: The Nudj Fitness Specialist is a key member of the Nudj lifestyle management team and is responsible for clinical fitness baselines and assessments, assigning and modifying exercise and functional fitness activities to promote behavioral change and improve the physical health of patients. Under the supervision of the Assistant Director of Cardiometabolic Services, the Fitness Specialist will work with patients in 1:1 and group settings to motivate and nudge them to reach their identified goals. Duties \& Responsibilities * Screen and assess the functional mobility of patients to determine if the patient is appropriate for the fitness pathway, utilizing metric\-based assessments to monitor progress and adjust intervention to support optimal patient outcomes and progression. * Assign and modify the exercise program for individual patients based on their goals, needs, progress, and continuous assessments. * Create and modify treatment plans and exercise prescriptions for patients with pain, injury, or mobility limitations. Patients will be followed virtually, with live telemedicine visits monthly. * Responsible for maintaining all aspects of case management, ensuring compliance and program prescription. * Document and track patient progress, treatment recommendations, and clinical outcomes in EHR and other required systems. * Facilitate and lead group educational and movement\-based fitness classes. * Participate in regularly scheduled team meetings and caseload reviews. Qualifications: * Bachelor's degree in Exercise Science, Exercise Physiology, Kinesiology or similar field required, Masters preferred. * Minimum of 2 years of recent experience working as an Exercise Physiologist in a Cardiac Rehab or equivalent Program. * Clinical certification in o
