**Job Description** **About Bloom360** Bloom360 is a telehealth company building a new model for primary care. Patients pay a membership and they’re supported by an integrated care team, which includes their primary care physician (you!), dietitian, physical therapist, and Concierge Health Assistant to help patients not only be well, but stay well. We combine clinical care with technology and continuous support to help people make lasting changes to how they live, move, and eat. **The Opportunity** We are looking for an ambitious, independently licensed Family Medicine Physician to join the Bloom360 team. This is a telehealth role where you will be working directly with members on preventive care, chronic disease management, and primary care. You will collaborate closely with our dietitian and physical therapist and help shape how medicine gets delivered at Bloom360\. **What You'll Do** * Conduct virtual primary care visits covering preventive health, chronic disease management, and acute concerns within the context of each patient's full medical history * Develop and manage individualized care plans that are evidence\-based and aligned with patient goals and clinical conditions * Deliver telehealth family medicine across full scope, including diagnosis, treatment, prescribing, and care coordination * Collaborate closely with the dietitian, physical therapist, and care navigator to integrate clinical care into the overall member experience * Support patients with metabolic conditions, cardiovascular risk, musculoskeletal concerns, and lifestyle\-related disease using a preventive and functional medicine lens * Apply behavior change strategies to help patients build sustainable health habits and overcome barriers to care * Educate patients on prevention, early intervention, and evidence\-based approaches to living healthier * Track progress using clinical and patient\-reported outcomes, adjusting care plans accordingly * Contribute to the development of Bloom360's clinical model, care protocols, and patient resources * Maintain accurate, timely documentation within the platform **What We're Looking For** Required Qualifications: * MD or DO with an active, unrestricted Family Medicine license * Licensed in 5 or more states— multi\-state licensure already in hand is strongly preferred * Candidates with a track record of standing up new state licenses quickly will also be considered * Telehealth experience required — comfort operating independently in a virtual care environment * Tech savvy with strong preference for demonstrated telehealth practice experience * Clear, confident communicator who can coach and educate members effectively over video * Independent contractor status — this role is structured as a 1099 engagement Preferred Qualifications: * Michigan licensure * Experience in preventive care, metabolic health, or lifestyle medicine * Familiarity with prescribing and monitoring care plans for chronic conditions such as diabetes, hy
