**About the Role**
We are seeking a Telemedicine Physician to join our innovative, multidisciplinary team committed to transforming chronic disease care through functional and preventive medicine. This fully remote position offers the opportunity to provide compassionate, patient\-centered care while helping patients improve their health through attentive listening, personalized treatment plans, and continuous support.
Our clinicians offer real solutions for patients with diabetes, pre\-diabetes, weight management concerns, and other chronic conditions. They help improve overall health outcomes while reducing reliance on medications through personalized, root\-cause care.
Our physicians collaborate closely with dietitians, health coaches, therapists, nurses, and other providers to deliver high\-quality virtual care. While this role offers flexibility through remote care delivery, physicians are expected to provide safe, timely, and reliable patient care during scheduled working hours.
**Responsibilities**
* Provide high\-quality, patient\-centered virtual care using a functional and preventive medicine approach.
* Manage a panel of patients through ongoing remote care and async follow\-up.
* Develop personalized care plans focused on preventing and improving chronic disease.
* Collaborate with physicians, dietitians, health coaches, therapists, and nurses in a multidisciplinary care model.
* Proactively address patient questions, lab reviews, medication management, and follow\-up needs in a timely manner.
* Utilize technology platforms, EHR systems, Google Workspace, spreadsheets, and other digital tools effectively in daily workflows.
* Participate in the development and refinement of clinical protocols, workflows, and patient care systems.
* Contribute to quality improvement initiatives and help optimize patient outcomes and engagement.
* Maintain accurate and timely documentation.
* Be adaptable and collaborative in a fast\-growing digital health environment.
* Support and mentor other clinical team members when appropriate.
**Ideal Candidate**
* MD or DO
* Active, unrestricted medical license.
* Licensed in multiple states preferred, with the following states highly preferred:Alaska, Arizona, Arkansas, Georgia, Idaho, Indiana, Iowa, Louisiana, Massachusetts, Minnesota, Mississipp
