Remote Care Coordinator – Triage & Transitions of Care (SNF / Value-Based Care) Overview We are seeking a compassionate and detail-orientedTriage & Care Coordination Specialistto support patients across Skilled Nursing Facility (SNF) and triage service lines. This role serves as a central communication hub between patients, providers, facilities, and interdisciplinary teams to ensure timely care coordination, improved patient outcomes, and seamless transitions of care.This is an excellent opportunity for someone who enjoys patient advocacy, problem solving, and working within value-based care environments while contributing to quality improvement initiatives. Key Responsibilities Patient Triage & CommunicationManage incoming patient and facility calls in a timely and professional mannerDocument interactions accurately within the electronic health record (EHR)Respond to voicemail, electronic, and fax communications within program turnaround standardsEscalate urgent clinical concerns to providers according to established protocolsCare Coordination & Patient AdvocacyServe as liaison between patients, providers, SNFs, hospitals, and community partnersAssist patients with navigating healthcare services and accessing resourcesSupport follow-up on orders, care plans, and provider recommendationsIdentify barriers to care and promote patient self-managementQuality & Operational SupportTrack quality and program metrics using internal tools and dashboardsHelp refine workflows to improve care transitions and reduce gaps in careParticipate in quality improvement and patient satisfaction initiativesCollaboration & ComplianceParticipate in interdisciplinary meetings and case discussionsMaintain HIPAA compliance and regulatory documentation standardsBuild professional relationships with internal teams and external partners Qualifications 2+ years of healthcare, care coordination, or patient support experienceSNF, post-acute, managed care, or value-based care experience preferredStrong communication and documentation skillsAbility to manage multiple priorities in a fast-paced environmentBachelor’s degree preferredBilingual Spanish preferred Compensation & Benefits Compensation:$16.90 to $23 per hourBenefits:Medical, dental, vision, retirement, paid time offSchedule:8 hour shifts, 12 hour shifts, weekends, overnights, weekdaysWork Setting:Remote#MASC104 Originally posted on Himalayas
