Raya Health is a care management company that partners with primary care clinics to deliver fully managed chronic care services for Medicare patients. We run Advanced Primary Care Management (APCM), Chronic Care Management (CCM), and Remote Patient Monitoring (RPM) programs on behalf of our clinic partners — handling enrollment, patient outreach, monitoring, and care coordination so providers can focus on practicing medicine. As we expand, we're looking for compassionate, detail\-oriented care coordinators who want to make a real difference in patients' lives from the comfort of their home.
The Role
As a Virtual Care Coordinator, you'll be the consistent, friendly point of contact for the Medicare patients enrolled in our care programs. You'll guide patients through enrollment, monitor their health between office visits, help them understand and manage their chronic conditions, and coordinate closely with their primary care teams. This is a fully remote position for someone with a clinical background who enjoys building relationships with patients over the phone and through digital tools.
Responsibilities
* **Patient enrollment:** Contact eligible Medicare patients to explain our care management programs, answer questions, address objections, and complete enrollment in APCM, CCM, and RPM services.
* **Remote patient monitoring (RPM):** Review incoming readings from connected devices (blood pressure cuffs, glucometers, scales, etc.), flag out\-of\-range values, follow up with patients, and escalate clinically significant findings to the care team.
* **Care management \& chronic care management (CCM):** Conduct regular check\-in calls, help patients adhere to their care plans, track progress between visits, document time and interventions, and support medication adherence and follow\-up.
* **APCM support:** Assist with the workflows and documentation that keep patients active and well\-served in our Advanced Primary Care Management program.
* **Patient education:** Create clear, easy\-to\-understand patient education handouts and discuss health conditions, lifestyle changes, and self\-management strategies with patients in plain language.
* **Documentation:** Accurately maintain patient records in the EHR, log all patient interactions, and ensure compliance with program and billing requirements.
* **Care team coordination:** Communicate with primary care providers and clinic staff to relay patient concerns, close care gaps, and keep care plans up to date.
Required Qualifications
* Experience as a Medical Assistant (clinical or administrative MA background).
* A reliable work\-from\-home setup, including a computer and stable internet connection.
