RPM/CCM/BHI Care Manager

Our solutions enhance engagement, prevent complications, and improve outcomes across both chronic and preventive care programs. **Job Summary** We are seeking compassionate and motivated Certified Medical Assistants, Licensed Practical Nurses (LPNs), and Registered Nurses (RNs) to join our virtual care coordination network. This role plays a vital part in supporting patients with multiple chronic conditions by ensuring continuity of care, proactively monitoring health data, and improving long\-term health outcomes. In this hybrid contract position, you will enjoy complete professional autonomy and the independence to manage your own schedule and direct your day\-to\-day workflow to deliver high\-quality virtual patient support. Our specialized programs are designed to provide ongoing remote care for patients between their regular provider visits. The goal is to help patients successfully navigate their chronic care programs, prevent complications, and improve their overall quality of life. **Responsibilities** * Conduct structured patient check\-ins, enrollment support, and monthly touchpoints per program cadence. * Support patient adherence through data review, device reading checks, reminders, and barrier identification. * Assist patients with remote program onboarding and basic technology troubleshooting. * Document patient interactions, actions taken, and next steps clearly, thoroughly, and consistently within the care platform. * Coordinate and route patient\-reported alerts or concerns using established protocols and closed\-loop handoffs. * Maintain strict HIPAA\-aware communication and secure handling of all patient information. * Contribute to quality improvement by providing practical feedback on documentation standards and software workflows. **Required Qualifications** * Hold an active MA certification or an active LPN/RN license. * Exceptional phone presence and patient\-facing verbal and written communication skills. * High level of comfort, patience, and empathy when speaking with senior and geriatric populations telephonically. * Consistently strong documentation habits with high attention to detail. * Comfort navigating care management software platforms, electronic registries, and structured clinical workflows. * Process\-driven and self\-directed mindset with the ability to independently meet patient outreach timelines. **Preferred Qualifications** * Prior experience handling RPM/CCM/RTM/BHI workflows or utilizing remote care management platforms. * Bilingual fluency (Spanish/English) is a strong operational plus.
