In addition to direct clinical care, the PMHNP serves as the GUIDE Provider of Record for Avail Health's participation in the CMS GUIDE (Guiding an Improved Dementia Experience) model, providing clinical oversight and serving as the authoritative clinical voice within the program's dementia care framework. You'll work closely with the LCSW, Dementia Care Navigator, and Care Coordinator, supported by an ambient AI scribe and technology-enabled workflows designed to reduce administrative burden and support high-quality virtual care delivery. This is a foundational clinical role in a newly launching program.
