Responsibilities include contacting caregivers to confirm patient visits, ensuring they have the correct plan of care, sending documentation when needed, and accurately documenting all interactions in the EMR system.This position is remote and offers you the flexibility of working from home.Key Responsibilities:Contact caregivers prior to or during scheduled visits to confirm arrival at the patient’s homeEnsure caregivers have the current and appropriate patient Plan of CareEmail or securely transmit care plans and related documentation to caregivers when necessaryDocument all communication, follow-ups, and care coordination activities accurately in the EMR systemEscalate patient care concerns, missed visits, or compliance issues to the appropriate clinical supervisorMaintain HIPAA compliance and patient confidentiality at all timesSupport continuity of care and timely coordination between field staff and office personnelAssist with basic care coordination and administrative nursing tasks as assignedQualifications:Registered Nurse with New York License - USRN - NYFluency in Cantonese and/or Mandarin and English requiredPrevious experience in Home Health, Care Coordination, Case Management, or Remote Nursing preferredStrong verbal and written communication skillsExperience using EMR/EHR systemsExcellent organizational and documentation skillsAbility to work independently in a remote environmentProficient with email, phone communication, and basic computer systemsService Fee:Industry standards applicable to the state depending on work experience and level of expertise.Hours:US Eastern Standard Time (EST) - 9am to 6pmWhy Limitlessli?We embrace the flexibility and convenience of a remote working environment, and you will collaborate with an international team while contributing to our growing business, all from the comfort of your home.Essential requirements:You will need to have some essential tools – a reliable computer and noise-canceling headset, a second monitor for enhanced productivity, and a stable internet connection.
