Our vision is revolutionizing how healthcare professionals produce, access, and utilize information and thus enabling them to focus on the core value of their work: patient outcomes.This is a non-bedside role for a Licensed Practical Nurse (LPN) to support patients enrolled in Medicare’s Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.Your Contribution:Create, update, and review patient care plans based on the provider’s plan of careConduct monthly telephonic check-ins with patients to assess health status and medication adherenceReview RPM synced vital signs and escalate concerns to the provider when appropriateCoordinate follow-ups, scheduling, and documentation updatesEnsure all documentation meets Medicare CCM and RPM compliance guidelinesAccurately document all patient interactions within multiple EHR systemsYour Qualification:Active LPN License (Multistate Nurse Compact License Required)Strong verbal and written communication skillsMinimum 1 year in related Chronic Care Management servicesExperience working with EHR systemsAbility to work independently with minimal supervisionReliable high-speed internet and a quiet, HIPAA-compliant workspaceWhat you can expect from us:Purpose: Become a part of a significant initiative.
