At POD Health, we strive to make healthcare personal, proactive, and accessible for all. **Position Title**: Nurse Practitioner **Reports To**: Senior Director of Compliance \& Clinical Operations **Location:** Onsite in our Tampa Office, Monday – Friday \| 9:00 AM – 5:00 PM \| **Job Summary:** The Nurse Practitioner (NP) will play a crucial role in delivering comprehensive healthcare services in both telehealth and in\-person environments. You will manage chronic and acute conditions, oversee patient monitoring through RPM, and collaborate with interdisciplinary teams to provide patient\-centered care. This role also supports CCM and CHI initiatives, ensuring ongoing care for chronic disease management and improving community health outcomes. **Key Responsibilities:** * **Patient Assessment:** Conduct comprehensive assessments of patients' health conditions, including physical examinations, medical histories, and diagnostic test results. * **Diagnosis and Treatment:** Diagnose and treat common acute and chronic illnesses, injuries, infections, and other medical conditions. Develop treatment plans and prescribe medications as needed. * **Preventive Care:** Provide preventive care and health maintenance services, including vaccinations, health screenings, and lifestyle counseling. * **Health Education**: Educate patients and their families about health conditions, treatment options, medications, and self\-care techniques. * **Care Coordination:** Collaborate with the Medical Director, Operations Manager, Care Navigators, and Medical Assistants to streamline patient care. * **Telehealth:** Offer seamless virtual healthcare consultations, referrals, and follow\-up care through our telehealth platform. * **Record Keeping:** Maintain accurate and up\-to\-date patient records and documentation in compliance with healthcare regulations and standards. * **Prescription Management:** Manage and monitor patients' medication regimens, including adjusting dosages and renewing prescriptions as needed. * **Compliance:** Ensure all documentation and patient care follow federal and state regulations, including HIPAA and telehealth guidelines. * **Community Health
