**Company overview:** Lumina Care is focused on unifying, coordinating, and managing care for geriatric patients in nursing facilities and at home. We offer a range of services to improve health outcomes and quality of life for patients, including after\-hours telehealth, transitional care, chronic care, remote patient monitoring, behavioral health and telehealth psychiatry. **Job title:** Primary Care Telehealth Clinician, Ohio and/or Texas licensure required. **Purpose/Summary:** Lumina Care is seeking an experienced Nurse Practitioner to join our Primary Care Telehealth team. This fully remote position provides comprehensive primary care services to residents of skilled nursing facilities using a secure telehealth platform with an on\-site facilitator assisting at the bedside. Providers manage chronic diseases, evaluate acute medical concerns, complete routine regulatory visits, and collaborate closely with facility staff and supervising physicians to deliver high\-quality, patient\-centered care. This position is ideal for clinicians who enjoy practicing independently while remaining part of a collaborative interdisciplinary team. Hours are Monday through Friday day shift hours. **Role \& Core Responsibilities:** * Conduct comprehensive telehealth evaluations for residents in skilled nursing facilities. * Perform Initial, Routine, Follow\-up, and Medically Necessary visits via secure video technology. * Evaluate and manage acute and chronic medical conditions. * Develop individualized treatment plans based on evidence\-based clinical practice guidelines. * Order and interpret laboratory studies, diagnostic testing, and imaging when clinically appropriate. * Prescribe, adjust, discontinue, and reconcile medications. * Identify opportunities for deprescribing and gradual dose reductions (GDRs) when appropriate. * Collaborate with facility nursing staff, therapists, consultants, specialists, and collaborating physicians. * Participate in care coordination to reduce avoidable emergency department visits and hospitalizations. * Support quality measures and value\-based care initiatives, including management of high\-risk residents. * Complete all documentation accurately and in real time using the electronic medical record with scribe support when available. * Maintain timely completion of orders and clinical documentation. * Participate in interdisciplinary care planning and quality improvement initiatives. * Complete assigned education and competency requirements. * Maintain compliance with all federal, state, payer, and company policies. * Support Lumina Care's mission, vision, and core values.
