Care Coordinator — On-Call - Weekends and PRN

**Sunstar Virtual Behavioral Solutions (SVBS)** **Remote \| Part\-Time \& PRN \| On\-Call \| Weekend Days \+ PRN All\-Shift Coverage** Sunstar Virtual Behavioral Solutions is seeking a **Care Coordinator** to join our growing virtual behavioral health team. Care Coordinators play an important role in helping patients access timely behavioral health services by coordinating referrals, gathering and communicating information, supporting hospital and community partners, and maintaining accurate documentation throughout the referral and placement process. **This is a remote, non\-clinical, on\-call position.** Care Coordinators are scheduled for designated **4\-hour on\-call shifts** and are expected to remain available and responsive throughout their assigned shift. Our current staffing priority is **Weekend Day on\-call coverage**, with an additional need for team members who are available to provide **PRN coverage across a variety of shifts when last\-minute coverage needs arise.** The role requires strong organization, attention to detail, sound judgment, and the ability to work independently in a fast\-paced behavioral health environment. About the Role Care Coordinators support the day\-to\-day coordination of behavioral health referrals and related administrative workflows. The role involves working with hospitals, treatment facilities, transportation providers, behavioral health programs, patients and support systems, and internal team members to help move referrals and care coordination needs forward. While Care Coordinators do not provide direct clinical care, they work within a behavioral health environment where they may encounter sensitive information, urgent situations, and circumstances that require discretion, careful judgment, and appropriate escalation. **Care Coordinators are expected to follow HIPAA and all applicable confidentiality requirements, use good judgment when handling patient information, and communicate or document information only through appropriate channels.** The ideal candidate is **organized, dependable, and resourceful**, with the ability to manage multiple priorities without losing track of the details. They communicate clearly, follow through on assigned tasks, work well independently, and understand when to use established processes versus when to ask questions or escalate a concern. Current Hiring Need **Our immediate staffing priority is Weekend Day on\-call coverage.** Candidates with consistent availability for **Saturday and/or Sunday daytime on\-call shifts** are strongly encouraged to apply. This means being willing and able to respond to last\-minute coverage requests for **any shift, including daytime, evening, weekend, holiday, and overnight shifts**, when available.
