The Cadence Health team is seeking a remote\-based Nocturnal Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey. In this role, you will lead clinical decision\-making with a strong emphasis on guideline\-directed medical therapy for heart failure, as well as adherence to evidence\-based guidelines for type 2 diabetes and hypertension, including proactive and asynchronous medication titration based on patient vitals, symptoms, and trends. You will oversee patients, manage clinical escalations, and collaborate closely with the patient's physician or provider to optimize treatment plans, adjust medications, and determine the appropriate level of care. This role requires a high level of clinical autonomy, strong judgment, and close collaboration with Registered Nurses and Patient Success teams to deliver timely, patient\-centered, and scalable remote care.
**Licensure Requirements**
* **Required:** Active Compact Multi\-State RN license
* **Required:** Active NP licensure in at least one of the following states: **TX, MI, NY, NJ, PA, CA**
* **Required:** Active NP licensure in two or more of the following states: **TX, MI, NY, NJ, PA, CA,** **AL, CT, HI, TN, WA, IA, IL, AK, AR, OK, OR, MT, MS, RI, or WI**
*Licenses must be active and in good standing at time of hire.*
**Available Schedules**
* Thursday, Friday, Saturday, and every other Wednesday, 7 PM–7 AM Eastern Time
* Sunday, Monday, Tuesday, and every other Wednesday, 7 PM–7 AM Eastern Time
**What you'll do:**
* Oversee the monitoring of patient vitals, symptoms, and labs in collaboration with our team of RNs and MDs.
* Manage the patient's treatment plan and medications – with an emphasis on initiating and titrating guideline\-directed medical therapy (GDMT) for Heart Failure and other appropriate guidelines for T2D and Hypertension.
* Respond to patient escalations identified via RPM – i.e. abnormal vitals, symptoms, and labs.
* Lead regular virtual check\-ins with patients to review labs and GDMT optimization and adherence.
