You’re comfortable practicing independently, curious and adaptable, and motivated by helping patients achieve meaningful improvements in daily life.You thrive in virtual care settings and value collaboration, autonomy, and purposeful work.ResponsibilitiesConduct comprehensive intake evaluations for new patients, identifying unmet needs, goals, and limitationsDevelop and support individualized care plans in collaboration with a multidisciplinary care teamProvide education to patients and referring clinicians on survivorship-focused care Support and guide interdisciplinary team members in patient treatment planningCommunicate with patients’ oncology teams and primary care providers as appropriateDeliver compassionate, patient-centered care via a secure telehealth platformRequirementsCurrent licensure as a Physician Associate or Nurse Practitioner (U.S.)Five or more years of experience in oncology or palliative care (preferred)Ability to work independently and problem-solve in a dynamic virtual environmentStrong communication skills and exceptional ability to build rapport with patients and colleaguesHighly compassionate, patient-centered approach to carePrivate, HIPAA-compliant residential workspace for telehealth visitsHigh-functioning personal computer with webcam, reliable high-speed internet, and smartphoneComfort using cloud-based tools including G-Suite, electronic medical records, and VOIP systemsSolid understanding of HIPAA, HITECH, and patient confidentiality requirementsSupport & Professional DevelopmentWe are committed to clinician success and well-being.
