Virtual Referral Coordinator / Outreach AssistantLean On Me Senior Services, LLCRemote PositionPay: $8\.00 – $10\.00 per hour Lean On Me Senior Services, LLC is seeking a motivated and professional Virtual Referral Coordinator / Outreach Assistant to assist with facility outreach and referral development for our non\-medical companionship and behavioral health support services. This position focuses heavily on outbound calling to hospitals, rehabilitation centers, nursing facilities, clinics, senior living communities, assisted living facilities, behavioral health facilities, and other healthcare organizations to help build referral relationships and generate new client opportunities. Responsibilities * Make outbound calls daily to: * Hospitals * Clinics * Nursing homes * Rehabilitation facilities * Assisted living communities * Senior living facilities * Social workers and case managers * Behavioral health organizations * Introduce Lean On Me Senior Services and explain services offered * Speak with admissions departments, social workers, discharge planners, and case managers * Follow up with referral sources professionally * Collect referral contact information and email addresses * Document call outcomes and notes accurately * Maintain outreach tracking spreadsheets and CRM systems * Send follow\-up emails and referral information * Schedule calls or meetings when needed * Assist with intake coordination and administrative tasks * Meet outreach and call activity expectations daily Services You Will Discuss * Non\-medical companionship * Wellness visits * Senior support services * Transportation accompaniment * Emotional wellness support * Behavioral health group sessions * LPC/LCSW support services * Post\-discharge support Qualifications * Strong English communication skills * Comfortable making outbound calls daily * Professional phone etiquette * Organized and detail\-oriented * Ability to work independently remotely * Reliable internet connection required * Healthcare outreach or call center experience preferred * Customer service or sales experience is a plus Preferred Experience * Healthcare outreach * Medical scheduling * Intake coordination * Referral development * Home care or senior care experience * CRM or spreadsheet tracking systems Compensation * $8\.00 – $10\.00 per hour * Pay based on experience, communication skills, and performance Schedule * Remote position * Flexible scheduling available * Part\-time To apply, please submit your resume and a brief introduction explaining your experience with phone communication, outreach, or healthcare support services.
