To be considered for this role, candidates must live within the designated territory. Reliable transportation, a valid driver’s license, vehicle insurance, a dedicated home workspace with high\-speed internet, and an active **CMA (AAMA), CCMA (NHA), NCCT, (NCMA), NAHP (NRCMA) or RMA (AMT)** certification are required. This position requires full\-time availability, Monday through Friday between the hours of 8:00 am \- 5:00 pm. If you love the idea of mixing patient interaction, clinical support, remote monitoring, and teamwork, then this role is for you! **What does this role look like?** This is a versatile, people\-focused, mission\-driven role where you: * Travel regularly within your assigned territory to visit long\-term care facilities. * Work directly inside facilities to perform wellness checks and clinical assessments. * Monitor patient vitals remotely from your home office to ensure continuity of care. * Troubleshoot devices to keep our technology running smoothly. * Bring organization, compassion, and professionalism to both physical and virtual healthcare environments. **Responsibilities** **Onsite Patient \& Facility Support** * Work directly inside long\-term care and senior care facilities, conducting wellness checks on each patient at least twice monthly per facility. * Perform full vitals assessments using MedStreamline’s mobile application, ensuring accuracy, consistency, and quality. * Build trust and meaningful relationships with residents, nurses, and facility leadership. * Ensure residents feel supported, comfortable, and cared for while serving as a consistent, professional presence. **Remote Monitoring \& Clinical Oversight** * Monitor assigned patients’ vitals via MedStreamline’s remote platform. * Identify abnormal readings and escalate critical alerts to Nurse Practitioners and physicians promptly. * Maintain a dedicated, HIPAA\-compliant home workspace to ensure patient privacy and data security. * Participate in virtual meetings and departmental updates to stay aligned with the broader clinical team. **Hands\-On Support \& Technical Readiness** * Troubleshoot and inspect MedStreamline’s devices inside facilities to ensure all equipment is functioning properly. * Provide basic device training to facility staff as needed to support smooth workflows. * Ensure all documentation is completed accurately and timely according to MedStreamline’s compliance standards. **Candidate Profile** **Required** * Active CMA (AAMA), CCMA (NHA), or RMA (AMT) certification. * 1\+ years of experience in patient care, long\-term care, or clinical support. * Reliable personal transportation, a valid driver's license, and vehicle insurance for
